There are so many options for how you showcase your child(ren). I have included just a few samples to give you some ideas. Don't feel that your family's portfolio must live up to some ideal or look just like another family's. Some keep everything digitally, some scrapbook style, and others keep everything in boxes/piles. Do what works best for your family and satisfies PA law.
You can purchase a planner designed for homeschooling or simply purchase a regular notebook.
Some people like to keep track of things in binders (and some of us decorate them).
I found this free simple attendance tracker online. I later created another one based off of this one.
I once used the calendar pages of this planner I purchased to keep track of the number of days.
The law requires homeschool families to keep a list (by title) of reading materials used. I usually listed my textbooks separately from my daughter's list of chapter books.
List of books read or textbooks used may be easily handwritten in a notebook or journal. Even the older students can help with this.
Some like to use planners for recording student activities/lessons. This one is a template from MS Word.
Dated records of what is planned OR what was done may even be handwritten and kept in a notebook.
Including programs, brochures, and photos is a great way to help show your evaluator other activities in which your child participated.
