You will need to submit a notarized affidavit prior to starting and annually between July 1 and August 1. Feel free to use one of the sample forms I have included below.
Along with your affidavit, you will need to submit an "outline of proposed education objectives by subject area" each year. This is simply an outline of what you want your child to accomplish during the year. It demonstrates that you have educational goals for your child. I have a few samples for both elementary and secondary below for you. It doesn't need to be detailed. I suggest being general, so that the district is satisfied that you have basic objectives and you have more time to develop any necessary details for your own planning.
You must show that your child has had proper immunizations or documentation of exemption. I have included a form for exemption. You should be able to simply ask your child's doctor for paperwork to confirm your child is up to date on immunizations and/or wellness checkups. I did find a physician's report form for school age children. You are welcome to use it if you find it helpful. It is attached below as a downloadable form.
Things get a little more specific when the kids enter high school. I am not listed with any of the organizations that have diploma programs. However, if you wish to provide your own diploma (Supervisor Issued Diploma), I have some goodies for you. I have included a graduation checklist, as well as information on supervisor issued diplomas.
